In this pivotal role, you will work closely with clients and caregivers, developing personalized support strategies reflecting individual goals. Utilizing comprehensive assessment tools and fostering effective communication, you will facilitate access to crucial services and monitor client care budgets.

Key Responsibilities:

• Interpret standardized assessment tools for client evaluations

• Build therapeutic rapport and support plans

• Assist clients with budget management

• Coordinate Family Home support services

• Advocate for additional resources when necessary

Requirements:

• University degree or diploma in a related field

• Minimum 3 years’ experience in case management

• Knowledge of community resources

• Exceptional communication and time management skills

• Valid driver’s license with personal vehicle access

Deliver high-quality services and advocate for clients' needs in the Family Home Program.
