Become a Mental Health Support Worker with CMHA Peel Dufferin. This full-time role combines mental health services and system navigation for transitional age youth navigating complex challenges.
This position will serve youth aged 16-24, emphasizing the development of comprehensive care plans and assessing their needs in a supportive environment. You’ll engage with community resources and provide crisis support, ensuring youth receive the help they need to thrive. A hybrid work model reflects the needs of clients.
Key Responsibilities:
• Facilitate coordinated access to community supports
• Implement case management tasks and interventions
• Conduct assessments and monitor youth progress
• Advocate for youth access to services and support
• Maintain accurate and timely documentation of interventions
Requirements:
• Educational background in Social Work or related fields
• Minimum 2 years in transitional support or case management
• Strong skills in interpersonal communication and advocacy
• Fluency in a second language is an asset
• Valid driver's license and clear police check
