Part-Time Service Coordinator

Join Brampton Caledon Community Living as a part-time Service Coordinator supporting individuals with intellectual disabilities. Help families navigate resources and coordinate community services effectively.

This contract role lasts up to six months and seeks an experienced Service Coordinator. You will assist families in accessing services and liaise with service providers, schools, and agencies to reach specific goals for individuals with disabilities. Flexibility for evening and weekend work is essential to meet client needs.

Key Responsibilities:

• Assist individuals with intellectual disabilities and families

• Coordinate with service providers and community agencies

• Promote family capacity and resource access

• Develop individual support plans based on needs

• Facilitate communication between families and agencies

Requirements:

• Bachelor’s degree or DSW/SSW from accredited institution

• 2-3 years of Service Coordination experience

• Knowledge of relevant community resources and services

• Strong interpersonal and communication skills

• Valid Class G driver’s license and reliable vehicle
