SERVICE COORDINATOR
Contract up to Six (6) Months
Brampton Caledon Community Living, a recognized leader in developmental services supporting over 2000 individuals and their families, is seeking a part-time Service Coordinator to join our team.
The Service Coordinator will assist individuals with an intellectual disability and their families to access resources and services, promote family capacity, and liaise and coordinate support with other service providers, schools, and community agencies to achieve identified goals.
Qualifications/Skills Required
A Bachelor’s degree from an accredited university in a related discipline is strongly preferred or a DSW or SSW from an accredited community college.
2-3 years of experience with Service Coordination
Experience supporting families with a family member with an intellectual disability, and knowledge of the resources and services relevant to families caring for a person with an intellectual disability.
Knowledge about relevant resources and services in the community, the developmental service system, developmental service legislation, government funding and resources, and the capacity to consent to and substitute decision‑making rules and regulations.
Knowledge of Special Education legislation, policies, and procedures
A proven track record in planning and working with families.
Strong interpersonal skills: ability to express and professionally present oneself; strong written and verbal communication skills; sound decision‑making skills.
Demonstrated understanding of Social Role Valorization and Personal Outcomes Measures
Demonstrated ability to work effectively as a team member.
The flexible schedule involves occasional evenings and weekends.
Must-Have
Valid driver’s license Class G and a reliable car
Acceptable Driver’s Abstract and personal liability car insurance
Current Standard First Aid/CPR & CPI
Proficiency in Computer skills
Fluency in French is an asset
