Housing Support Case Manager Position

Overview

Become a vital part of CMHA Peel Dufferin as an Intensive Case Manager supporting homeless individuals in Peel Region. This full‑time role features shifts from 11am to 7pm in a permanent position. In this rewarding role, you will provide comprehensive housing support services, focusing on individuals who have a portable housing subsidy.

Your responsibilities will include assessing needs, creating care plans, and offering vital transitional support over a period of up to 18 months. Active collaboration with multidisciplinary teams and community resources will be essential for your success in helping clients achieve stability.

• Manage housing transitions through street outreach
• Develop and implement individualized care plans
• Provide targeted community support for clients
• Work collaboratively with other professionals
• Facilitate group activities and liaise with landlords

• Degree or diploma in a human services field
• Two years of relevant experience in case management
• Solid understanding of mental health and recovery principles
• Valid Canadian driver’s license required
• Second language skills, including ASL, are preferred
