Join Well Fort’s Bloom Team as an Outreach Worker in Brampton, ON, earning between $27.48 - $32.44 CAD per hour. Engage with communities at risk for Hepatitis C and HIV/AIDS while delivering essential outreach services.
This full-time, permanent role requires dedication to improving access to support, education, and referrals for vulnerable populations. As an Outreach Worker, you'll engage in low-threshold outreach, design educational workshops, and coordinate a Peer Support Worker Program. This position is ideal for those passionate about community engagement and public health.
Key Responsibilities:
• Conduct street outreach for community education
• Design and deliver HIV/HCV workshops
• Provide point of care testing and counseling
• Facilitate referrals to Well Fort services
• Participate in public health policy development
Requirements:
• Post-secondary education or relevant experience
• Lived/living experience with target populations
• Experience in trauma-informed outreach
• Valid Ontario Driver’s license required
• Ability to work independently in a team
