Key Responsibilities:

• Conduct eligibility assessments for hospice services

• Facilitate admission processes in a timely manner

• Prioritize patient advocacy and access to care

• Foster strong relationships with patients and families

• Actively contribute to mission-driven initiatives
Requirements:

• Associate’s degree or equivalent nursing education

• 6 months to 1 year nursing experience preferred

• Current valid Florida RN license

• Valid Florida Driver’s License

• Dedicated to providing dignified patient care
