Join the Department of Families as a Supports Intensity Scale Facilitator. Reporting to the Program Manager, you’ll use social work methods to conduct comprehensive assessments of Community Living Disability Service clients. This role requires a combination of interpersonal skills and independent judgment to support clients facing cognitive and behavioral challenges while collaborating with their networks.
Key Responsibilities:
• Conduct multidisciplinary support needs assessments for clients
• Apply social work theory and methods during evaluations
• Utilize interviewing skills to administer the Support Needs Scale
• Work as part of a service delivery team on key assessments
• Develop intervention and funding strategies for clients and families
Requirements:
• Post-secondary education in social services (BSW preferred)
• Experience with adults with intellectual disabilities
• Knowledge of Community Living Disability Services
• Strong organizational and conflict resolution skills
• Proficiency in MS Office applications
