Life's Journey Inc. is seeking a dedicated Case Worker for their Rural Connections program in Brandon, MB. This full-time permanent role focuses on supporting individuals through direct assistance and effective service planning.
As a Case Worker, you will engage with program participants, ensuring a holistic and respectful approach to support services. This position requires a strong ability in communication, problem-solving, and a commitment to maintaining community housing. Collaborate with Clinical Case Managers and the support team to enhance individual service plans and implement effective programming strategies.
Key Responsibilities:
• Provide direct support to individuals in the program
• Track and assist with service planning and follow-ups
• Liaise effectively with community resources and external agencies
• Mentor individuals through guidance and support
• Maintain comprehensive records and reporting documents
Requirements:
• Minimum two years of outreach support experience
• Familiarity with youth wellness and cultural programming
• Knowledge of community resources and housing processes
• Effective communication skills and adaptability
• Relevant education or experience in social services
