Hawthorne Centers for Rehabilitation is dedicated to serving our residents with high-quality, affordable, and accessible health services. Located at 851 W Lumsden Rd, Brandon, FL 33511, we strive to ensure our residents have the resources and support needed to reach their highest level of functional independence.
Responsibilities include:- Admissions, discharges, and transfers of residents
- Medication requisitions with pharmacy and physicians
- Medication administration
- Following infection control, emergency protocols, and other center policies
- Collaborating with the interdisciplinary team to provide optimal care
- Identifying and reporting abnormal changes in residents’ conditions
- Delivering nursing care for long-term or rehabilitative needs
- Collecting patient/resident data, observations, and reports
- Implementing and evaluating care plans according to regulations
- Supervising and directing caregiving staff in specific situations
- Maintaining documentation and equipment knowledge
- Ensuring confidentiality and respecting patient/resident rights
- Licensed Practical Nurse (LPN) license issued by the State
- CPR certification required
- Two years of prior nursing experience preferred
