About Us:

Hawthorne Centers for Rehabilitation is dedicated to serving our residents with high-quality, affordable, and accessible health services. Located at 851 W Lumsden Rd, Brandon, FL 33511, we strive to ensure our residents have the resources and support needed to reach their highest level of functional independence.

Responsibilities include:
  • Admissions, discharges, and transfers of residents
  • Medication requisitions with pharmacy and physicians
  • Medication administration
  • Following infection control, emergency protocols, and other center policies
  • Collaborating with the interdisciplinary team to provide optimal care
  • Identifying and reporting abnormal changes in residents’ conditions
  • Delivering nursing care for long-term or rehabilitative needs
  • Collecting patient/resident data, observations, and reports
  • Implementing and evaluating care plans according to regulations
  • Supervising and directing caregiving staff in specific situations
  • Maintaining documentation and equipment knowledge
  • Ensuring confidentiality and respecting patient/resident rights
Qualifications:
  • Licensed Practical Nurse (LPN) license issued by the State
  • CPR certification required
  • Two years of prior nursing experience preferred
