• Daytime hours, Monday through Friday, with flexible visit times

• Assess/reassess patients and develop personalized care plans
• Collaborate with providers and communicate changes promptly
• Manage payment sources and ensure ongoing authorizations
• Educate patients and caregivers; initiate discharge planning on admission
• Complete documentation within 24 hours, including changes in condition and follow-up
• Evaluate patient progress and act promptly
• Oversee LPNs and Home Health Aides
• Participate in case management meetings and document accordingly
• Schedule patients independently per care plan
• Attend staff meetings and mandatory trainings
• Follow policies and ensure accurate OASIS documentation

Education: Nursing Diploma or Associates Degree in Nursing;
Bachelor's preferred

Experience: 2 years as a Registered Nurse, home health experience preferred

Licensure: Valid Connecticut RN license or eligibility

Skills: Excellent clinical, verbal, written, and organizational skills; ability to work independently; strong team collaboration skills

Physical Demands: Ability to perform physical tasks including lifting and extensive standing

Coverage area:
Guilford and Branford; rotating weekends and holidays
