The Registered Nurse will work within an interdisciplinary team focusing on patient-centered and trauma-informed care. You will conduct nursing assessments, chronic disease management, and care coordination. Join a team committed to promoting health equity and addressing barriers for Indigenous communities.
Key Responsibilities:
• Provide nursing assessments and triage for patients
• Conduct and communicate results from blood work
• Administer immunizations and preventative care interventions
• Develop individualized care plans collaboratively
• Facilitate case management and continuity of care
Requirements:
• Bachelor of Science in Nursing or equivalent
• Current registration with the College of Nurses of Ontario
• 3-5 years of nursing experience required
• Knowledge in chronic disease management and Indigenous health
• Valid driver's licence and reliable vehicle needed
