As a key member of the interdisciplinary team, the Registered Nurse will offer comprehensive primary care, care coordination, and chronic disease management. This role involves assessing patient needs, triaging, and providing preventative care and education. You will also facilitate care plans and ensure timely follow-ups for patients across diverse backgrounds.
Key Responsibilities:
• Conduct nursing assessments and triage for patient needs
• Implement and monitor individualized care plans
• Administer immunizations and preventative care treatments
• Support chronic disease management and education
• Maintain documentation in the electronic medical record
Requirements:
• Bachelor of Science in Nursing or equivalent degree
• Current registration with the College of Nurses of Ontario
• 3-5 years of nursing experience, preferably in primary care
• Certification as a Diabetes Educator or willingness to obtain it
• Valid driver's license and access to a reliable vehicle
