As a vital member of the Primary Care Team, the RN Outreach plays a key role in delivering patient-centered services to Indigenous individuals and families. Your work includes nursing assessments, chronic disease management, triage, and preventive care, with a focus on outreach initiatives. Collaborate with interdisciplinary teams to enhance access and continuity of care across various settings.
Key Responsibilities:
• Conduct thorough nursing assessments and follow-up care
• Manage chronic disease and provide diabetes education
• Facilitate care coordination and referrals
• Administer vaccines and monitor health outcomes
• Document care accurately in the electronic medical record
Requirements:
• Bachelor's degree in Nursing or equivalent
• Current registration with the College of Nurses of Ontario
• 3-5 years of nursing experience in primary care
• Basic Cardiac Life Support certification
• Valid driver's license and vehicle access
