Nurse Practitioner
Position
Grand River Community Health Centre (GRCHC) is hiring a Nurse Practitioner (NP) to provide primary health care services to individuals and families.
Key Responsibilities- Conduct comprehensive health assessments, diagnosis, treatment, and management of acute and chronic conditions.
- Develop and implement individualized care plans in collaboration with patients and interdisciplinary team members.
- Prescribe medications and other therapeutic interventions in accordance with scope of practice.
- Provide health education, counselling, and support to patients and families to promote overall well‑being.
- Collaborate with physicians, social workers, dietitians, and other health professionals to ensure coordinated care.
- Participate in quality improvement initiatives and health promotion programs within the community.
- Maintain accurate electronic medical records and documentation in accordance with regulatory and organizational guidelines.
- Registered Nurse (RN) with extended class certification (NP) in good standing with the College of Nurses of Ontario.
- Preferred 2 years of experience in primary care, community health, or a related field.
- Strong clinical skills with a focus on patient‑centred, evidence‑based care.
- Excellent communication, collaboration, and problem‑solving abilities.
- Ability to work independently and within an interdisciplinary team environment.
- Experience working with diverse and vulnerable populations is an asset.
- Proficiency in electronic medical records (EMRs) and documentation.
- Demonstrated experience in Community and/or Primary Care settings.
- Knowledge and proficiency in current, evidence‑based methods and practices of primary care delivery, with an emphasis on health promotion and harm reduction.
- Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills.
- Ability to work autonomously as part of a multi‑disciplinary team.
This position offers a competitive wage range of $56.97 to $68.67 per hour and benefits including HOOPP.
Hours and LocationHours:
primarily days, with some evening, weekend work as required.
