The Inter-professional Primary Care Team (IPCT) will work collaboratively to provide primary care services to registered Primary Care clients and community clients, in various settings (at the CHC, at partner agencies or outdoors in the community) with a specific focus on marginalized populations. The IPCT team will provide services such as access to chronic disease prevention and management, mental health and addiction assessment, referral and counselling, as well as primary care clinics and outreach primary care services.
The RN collaborates with members of the IPCT to plan, implement, coordinate and evaluate the programs and services provided to clients. The RN will utilize their clinical skills to identify client and community needs which will lead to the provision of individual services, clinics and group programs. The RN with support from administration and leadership, will organize and coordinate services specifically related to chronic disease management, routine health screenings, cancer screening and other clinics as identified.
This role would conduct assessments and physical examinations, health screening, cancer screening, administering immunizations and other injections, performing venipuncture, wound care, and any other nursing related duties that may be requested within the RN scope of practice. The RN will also work individually or with others on the team or in the community to provide education and support for chronic diseases such as diabetes, COPD, HTN, CAD, CHF etc.,
and chart according to college requirements in an EMR. The RN will support the evaluation process for this program.
Skills:
- Coordination of clinics and services for community clients
- Perform nursing skills (wound care, injections, suture/staple removal, overdose response, etc)
- Work collaboratively with the team and other system partners (primary care providers, walk-in clinics, other community partners) to develop, coordinate and execute care plans
- Demonstrated knowledge of mental health, concurrent disorders, treatment modalities, and harm reduction practices is an asset
- Strong communication (verbal and written), organization, and time management skills
- Demonstrated ability in conflict management and interpersonal relations
- Understanding of anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds
- Ability to work collaboratively within and across teams, developing and maintaining positive relationships with service providers
- Ability to work independently and in self-directed working conditions
- Able to work flexible hours primarily within a 5 day, Monday to Friday work week. Regular evening work 1-2 times per week.
- Undergraduate degree in nursing from a recognized university or combination of nursing diploma and additional relevant courses/training
- Current registration with the College of Nurses of Ontario
- 3-5 years nursing experience in a community setting or combination of community, hospital, and/or public health settings; experience in a primary care setting an asset
- Smoking cessation certificate, Certified Diabetes Educator, Spirometry Certificate preferred or willingness to complete with continuing education support
