Geriatric Emergency Nurse at BCHS

Provide specialized care as a Geriatric Emergency Management Nurse at Brant Community Healthcare System, supporting the needs of elderly patients in Brantford, Ontario. You will assess, coach, and collaborate with teams to enhance emergency services.

In this role reporting to the Clinical Manager of Geriatrics, you will conduct detailed assessments in the Emergency Department. Your expertise will help prevent unnecessary hospitalizations and ensure safe community discharges for geriatric patients. Collaboration with nurses across Ontario and local resources will strengthen care for this population.

Key Responsibilities:

• Complete assessments to identify elderly patient needs

• Coach ED staff in managing complex geriatric cases

• Collaborate with dementia teams for timely patient discharge

• Communicate findings to the interdisciplinary team

• Develop educational resources on elder care best practices

Requirements:

• Baccalaureate Degree in Nursing required, Master's preferred

• Registration with the College of Nurses of Ontario

• Minimum five years of geriatrics/emergency services experience

• Basic Cardiac Life Support certification required

• Completion of Gerontological Nursing Certificate mandatory
