Position: ParaMed Clinical Planner Opportunity

As a Clinical Planner at Para Med, you will play a vital role in onboarding new patients that need personal support services. Your responsibilities will include assessing care needs, developing tailored individual care plans, and ensuring a safe living environment for patients. You will also educate patients and their families about Para Med's policies and maintain comprehensive patient records throughout your work.

Key Responsibilities:

• Assess initial care requirements for new patients

• Establish individualized care plans based on assessments

• Conduct safety evaluations of patient environments

• Communicate scheduling parameters based on clinical needs

• Record and maintain accurate patient documentation

Requirements:

• Active RPN/LPN registration and relevant nursing experience

• Familiarity with healthcare legislation and community resources

• Strong written and verbal communication skills

• Excellent organizational and critical thinking capabilities

• Personal vehicle access for travel purposes
