Deliver personalized home health care in Brantford with Com For Care , using comprehensive assessments and skilled nursing interventions for patients at home.
As a Registered Nurse, you will develop individualized care plans and administer medications, treatments, and procedures. You will monitor patient progress, coordinate with physicians and families, and provide education. The position requires clinical assessment, critical thinking, communication, and independent caseload management.
Key Responsibilities:- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and procedures under physician orders
- Monitor and document health status, progress, and condition changes
- Coordinate care with physicians, families, and interdisciplinary teams
- Provide education on health management, medications, and care techniques
- Current Registered Nurse registration with the College of Nurses of Ontario
- Home health, community, or long-term care experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Valid driver’s license, reliable vehicle, CPR, and First Aid certification
