- Adult and Geriatric Patients

- Evaluate and Manage Adult and Geriatric Patients
- Perform Routine Primary Care Evaluations

- Conduct Follow-Up Patient Visits
- Develop and Manage Patient Treatment Plans
- Provide Post-Acute Care Services as Needed
- Evaluate and Manage SNF and Assisted Living Patients
- Coordinate Patient Care Across Clinic and Post-Acute Settings
- Respond to Telephone and Beeper Call Coverage
