Compensation

• Orientation: $70/hour
• Regular Hourly Rate: $120/hour
• Overtime: $160/hour
• Holiday Rate: $160/hour
• Callback Rate: $160/hour
• Holiday Night Call: $450
• Holiday Overtime: $200/hour

Practice Highlights

• Primarily outpatient clinical practice
• Average of 16 patients per day
• Adult and geriatric patient population
• Limited SNF and assisted living coverage
• Flexible scheduling options
• Approximately 1:10 call rotation
• Beeper/phone call coverage, averaging approximately one callback per day
• Seeking 2–3 providers
• No teaching or leadership responsibilities

Responsibilities

• Provide comprehensive primary care services to adult and geriatric patients.
• Conduct preventive health assessments and routine evaluations.
• Diagnose and manage acute and chronic medical conditions.
• Coordinate care and support continuity of care across outpatient and post-acute settings.
• Provide limited SNF and assisted living coverage as needed.
• Participate in the call rotation and respond to patient callbacks.
• Maintain accurate clinical documentation using Cerner/Oracle.

Qualifications

• Nurse Practitioner or Physician Assistant with appropriate Family Medicine certification or training.
• Board-certified or board-eligible candidates.
• Adult patient care experience required.
• Geriatric patient care experience required.
• Primary care experience preferred.
• Active Vermont license preferred.
• Current BLS and DEA required.
• Candidates with lapsed board certification will not be accepted.
• Candidates with minimal malpractice settlements may be considered on a case-by-case basis.

• Anticipated coverage start:
  December/January

• Approximately 1:10 call rotation
• Beeper/phone call coverage
• Average callback volume: approximately one call per day
