Job Title:
LVN/LPN - Licensed Vocational Nurse / Licensed Practical Nurse
- Administer medications and treatments as prescribed by the physician.
- Monitor and record patient vital signs.
- Assist with patient assessments and care planning.
- Collaborate with the healthcare team to ensure comprehensive patient care.
- Provide emotional support to patients and their families.
- Maintain accurate and up-to-date patient records.
- Adhere to all facility policies and procedures.
- Share information appropriately with members of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation.
- Participate in pre‑admission evaluation as requested by Program Manager and complete Admission Nursing Assessment on all new admissions to C/A Residential I & Partial Programs (Including ARCC).
- Conduct monthly medication renewals and review daily reports.
- Follow up on medical tests and labs, oversee completion of admission paperwork, and complete the medication portion of aftercare forms.
- Track medical errors.
