Job Title:

LVN/LPN - Licensed Vocational Nurse / Licensed Practical Nurse

• Administer medications and treatments as prescribed by the physician.
• Monitor and record patient vital signs.
• Assist with patient assessments and care planning.
• Collaborate with the healthcare team to ensure comprehensive patient care.
• Provide emotional support to patients and their families.
• Maintain accurate and up-to-date patient records.
• Adhere to all facility policies and procedures.
• Share information appropriately with members of the multidisciplinary team and other hospital department representatives through verbal reports and written documentation.
• Participate in pre‑admission evaluation as requested by Program Manager and complete Admission Nursing Assessment on all new admissions to C/A Residential I & Partial Programs (Including ARCC).
• Conduct monthly medication renewals and review daily reports.
• Follow up on medical tests and labs, oversee completion of admission paperwork, and complete the medication portion of aftercare forms.
• Track medical errors.

• Active LVN or LPN license in the state of Vermont.
• Psych/Behavioral Health experience – Required.
• Experience working with child and adolescent populations – Required.

• Attention to detail and strong organizational skills.
• BLS and CPI Certification.
