Description:
A Primary Care Post-Acute Care opportunity is available due to recent clinician departures.
The organization seeks Nurse Practitioners and Physician Assistants to provide outpatient primary care services and limited SNF/Assisted Living coverage.
Multiple engagement models are available, including locum tenens, locum-to-permanent, and permanent placement opportunities.
Coverage is anticipated to begin in December/January.
Flexible scheduling is available.
Call coverage is required with a call ratio of 1:10 and an average callback volume of approximately one call per day.
The practice setting is clinic-based, with less than 1.0 FTE in SNF Assisted Living coverage.
Care will primarily focus on adult and geriatric patients, with an average daily patient volume of 16 patients.
Clinical requirements include:
- Ability to manage adult and geriatric patient populations.
- Experience providing outpatient preventive and chronic disease management.
- Ability to provide care across clinic and post-acute settings.
Procedures involved are general primary care evaluation and management, preventive care services, chronic disease management, and post-acute care services as needed.
Documentation will be completed using EMR software.
This flexible opportunity is well-suited for candidates interested in a blend of primary care and post-acute medicine.
The ideal candidate will be comfortable caring for adult and geriatric patients in an outpatient setting while supporting limited SNF and assisted living responsibilities.
With multiple engagement models available and flexible scheduling, this position offers an attractive option for those seeking either short-term coverage or a long-term practice opportunity.
