Base pay: $22.00/hr - $24.00/hr

Bachelor’s degree in human services and prior case management experience preferred. A combination of relevant education and experience may be substituted for a degree. Must have reliable transportation and a valid driver's license.

• Collaborating with other community partners to create a person-centered plan of care.
• Monitoring and reassessing program service plans as required.
• Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
• Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
• Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
