Community Team Manager - Adult Mental Health Ty Illtyd

The Team Manager is responsible for the day-to-day operational management of the team, in partnership with clinical and admin leads and the Service Manager. We are looking for someone who is passionate about delivering excellent mental health services and can support team members to achieve their potential. Key responsibilities will include staff supervision, appraisal, recruitment and day to day management. Also ensuring that people have timely access to our services and receive treatment in line with our pathways.

You will promote person-centred care with a recovery focus, wherever possible.

The post holder will be part of the PTHB Mental Health Manager's On-Call Rota, which is a 12-hour on-call period approximately 15 times a year

This will be an opportunity to manage an enthusiastic, evidence-based team striving for excellence in working with colleagues who are passionate about promoting positive mental health within the community.

The working hours are Monday to Friday 9am-5pm, though this may change depending on the needs of the service.

• Ensure the comprehensive provision of a cost effective, accessible, appropriate and responsive Community Mental Health Team (CMHT) service to service users with complex mental health needs.
• Support the Adult Services Manager and Health Board/Local Authority personnel with the implementation of the Powys Dementia Plan, Mental Health National Service Frameworks, underpinned by a person-centred approach and develop policies and protocols to support the associated principles and values.
• Continue responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.
• Develop and coordinate a multi-professional approach providing evidence based mental health care to all those who use the service. This collaboration will harness an integrative agency approach so that physical, psychological and social care agencies are working together in providing holistic care to individuals who require its support.
• Responsible for the continued development of the CMHT and ensure service delivery is of a high professional standard, inline with Health Board policies and procedures.

• Qualified Health or Social Care Professional (RMN, Dip SW etc)
• Evidence of personal and clinical development
• Management experience / Understanding of management challenges
• Extensive knowledge of a wide range of mental health issues
• Knowledge of research and evidence-based interventions both new/historic. Able to underpin these with practice related experience
• Knowledge of current legislation
• Understanding of the Health & Social Care agenda
• Use of Carer involvement
• Knowledge of person-centred care
• Experience of specialist research in mental health AMHP training/Nurse prescribing
• Teaching qualification
• In-depth medication management skills, and ability to demonstrate how these are used

• Ability to make own transport arrangements for home visits/to travel between sites
