Specialist Ward

Experience:

Develop valuable expertise within elderly care, medical wards, and older adult mental health services.

Supportive Team Environment: Work alongside experienced pharmacy professionals committed to delivering outstanding patient care.

What you will do:

Work across specialist ward environments, including elderly care wards, medical wards, and older adult mental health services.

Undertake medicines reconciliation activities to ensure safe and effective medication management.

Complete patients’ own drugs assessments and support medicines optimisation initiatives.

Manage medicine discharge processes, helping to facilitate safe and timely patient discharges.

Supervise and support Pharmacy Assistants and other support staff where appropriate.

Provide ward-based dispensing support without routine commitment to hospital dispensary work.

Work collaboratively with pharmacists, nurses, medical teams, and other healthcare professionals to deliver high-quality pharmaceutical care.

Support the implementation and utilisation of electronic prescribing systems, including Better Meds, with training provided where required.

Maintain accurate records and documentation in line with legal, professional, and organisational requirements.

Contribute to patient safety initiatives and quality improvement activities within the pharmacy service.

Person Specification:

Current registration with the General Pharmaceutical Council (GPhC).

Qualified Pharmacy Technician with experience in hospital or ward-based pharmacy settings.

Experience undertaking medicines reconciliation and medicines management activities.

Full UK driving licence and access to a vehicle due to travel requirements between hospital sites.

Enhanced DBS for Adults.

Strong communication, organisational, and teamwork skills.

Ability to work independently and as part of a multidisciplinary healthcare team.
