About the RoleService Care Solutions are currently recruiting for an experienced Adults Social Worker – Disabilities to join the South Disabilities Operational Team at Powys County Council. This is an excellent opportunity for a qualified Social Worker with experience in Adults Social Care and/or Disabilities Services to join an established operational team and contribute to positive outcomes for adults and their families. The role offers flexible working arrangements, providing a good balance between professional responsibilities and flexibility.
Key Responsibilities- Undertake assessments of adults with care and support needs.
- Develop and review person-centred support plans.
- Work alongside individuals, families and carers to identify strengths, needs and desired outcomes.
- Safeguard adults and respond appropriately to safeguarding concerns.
- Work collaboratively with health professionals, providers and other partner agencies.
- Apply relevant legislation, policies and professional standards to social work practice.
- Maintain accurate and timely case records and documentation.
- Support individuals to maximise independence, choice and control.
- Participate in reviews and ensure appropriate care and support arrangements are in place.
The successful candidate will be a Social Care Wales registered and qualified Social Worker with relevant experience within Adults Social Care. You will ideally have:
- A recognised Social Work qualification.
- Current Social Care Wales registration.
- Experience working within Adults Social Care.
- Experience working with adults with disabilities is highly desirable.
- Knowledge of relevant legislation and safeguarding procedures.
- Strong assessment, care planning and review skills.
- Excellent communication and interpersonal skills.
- The ability to work effectively both independently and as part of a multidisciplinary team.
