Je gaat zelfstandig huisbezoeken uitvoeren en poli-afspraken maken, met als doel stabilisatie van mensen in crisis. Dit vereist een HBO-opleiding en zorgervaring. De functie biedt ruimte voor persoonlijke en professionele ontwikkeling en wordt ingeschaald in FWG 45.
