BuurtzorgT zoekt een ervaren GGZ-agoog voor het team Breda centrum west. Je komt te werken in een multidisciplinair en zelfsturend team dat volop in ontwikkeling is in Breda, met aandachtsgebieden in Centrum, Princenhage, Tuinzigt, Heuvel en Westerpark.
Je maakt deel uit van korte lijnen, veel afstemming en een open, lerende sfeer. Je start vanuit de krachten van de cliënt en zoekt samen met betrokkenen naar passende behandelopties, en levert richting aan de werkwijze van het team.
