Zintri zoekt een ambulant begeleider GGZ in West Brabant West/Oost voor kinderen, jongeren en volwassenen. Je helpt cliënten grip te krijgen op dagelijks leven, vergroot zelfredzaamheid en regie, met herstelgerichte zorg en samenwerking met netwerk en gemeenten.
Je maakt begeleidingsplannen, SMART-doelen en bewaakt voortgang. Je onderhoudt rapportage, dossiervorming en overdracht en werkt in een betrokken team aan goede cliëntenzorg.
