Wat ga je doen?
Als persoonlijk begeleider sta jij naast de bewoners in hun dagelijks leven. Je helpt ze om zo zelfstandig mogelijk te leven en stimuleert ze om echt mee te doen in de wijk. Samen met je collega's zorg je voor een veilige, vertrouwde en warme thuishaven.
Jij bent voor jouw bewoners het vaste aanspreekpunt. Je stelt samen met hen persoonlijke doelen op en voert de regie over het begeleidingsproces. Wat vindt iemand belangrijk? Waar wil hij of zij in groeien? Jij luistert, denkt mee en maakt het concrete. Je dag is lekker afwisselend:
- Je biedt dagelijkse zorg en begeleiding, afgestemd op wat iedere bewoner nodig heeft
- Je stelt zorg- en ondersteuningsplannen op, voert ze uit en evalueert ze
- Je signaleert ontwikkelingen, rapporteert en stuurt bij waar nodig
- Je gaat samen op pad, bijvoorbeeld om boodschappen te doen of een stukje te wandelen
- Je denkt in het team mee over zorginhoudelijke vraagstukken
