Wound Care Nurse (RN or LPN) | $45–$55/hr | Bremerton, WA

Port Washington Post Acute | Bremerton, WA | Full-Time

Are you the nurse everyone calls when a wound isn't healing? At Port Washington Post Acute, you'll own our skin and wound care program. You'll be the go-to expert who prevents pressure injuries, leads treatment plans, and shows how much healing makes a difference for our residents. What

You’ll Do
  • Lead the facility’s skin and wound care program, including assessment, treatment, and prevention of pressure injuries and other skin conditions

  • Perform wound assessments, staging, and measurements, and track healing progress

  • Partner with physicians, the Medical Director, and the Director of Nursing on treatment plans and referrals

  • Collaborate with the interdisciplinary team on care plans and skin-related assessments

  • Train nursing staff and CNAs on skin care, prevention techniques, and wound care best practices

  • Educate residents and families on skin care, especially before discharge home

  • Keep wound care supplies and treatment products stocked

  • Maintain accurate, timely documentation and support state survey readiness

  • Advocate for residents’ dignity, rights, and quality of care

What You’ll Need
  • Current, active Washington RN or LPN license

  • Experience in long-term care, skilled nursing, or post-acute settings

  • Wound care experience, including assessment, staging, and treatment

  • Wound care certification (WCC, CWCN, or similar) is preferred but not required

  • Strong communication skills with residents, families, and the care team

  • Ability to work independently and make sound clinical decisions
