Hiring Range: $22.94 - $28.10 Per Hour

This position follows a 92% compensation structure to align with the academic calendar year and scheduled working hours.

Kitsap Mental Health Services (KMHS) is seeking a dedicated and compassionate Care Coordinator to support students and families in our School Programs. In this role, you’ll work closely with a multidisciplinary team and school partners to provide care coordination, crisis support, and family engagement services that help students build stability and succeed. You’ll play an important part in connecting students to resources, supporting educational goals, and strengthening collaboration between schools, families, and providers.

Primary Responsibilities
  • Provide care coordination services for an assigned group of clients, including crisis intervention and case management, and collaborate directly with schools and district personnel.
  • Participate in interventions to support school expectations, including chronic absenteeism; identify barriers to academic, social, or emotional success and help create plans to address them.
  • Assist clients in identifying and building natural supports; schedule and facilitate client-centered team meetings.
  • Educate and support families and advocate for client needs; consult with families and community agencies such as DSHS, courts, housing authorities, and more.
  • Assist in developing, writing, implementing, evaluating, and revising treatment goals and assessments in collaboration with the treatment team, including completing the CANS assessment.
  • Support clients in accessing medical and dental care and building basic health self-management skills; coordinate psychiatric and medical care with the treatment team.
  • Document client progress according to agency policies to maintain accurate and timely records.
  • Help clients gain resources and education to promote autonomy and meet basic needs.
  • Provide information to clients about substance use and its impact on daily functioning; coordinate with internal and external substance use treatment providers.
Minimum Qualifications

EDUCATION:

Bachelor’s Degree in Psychology, social services, or behavioral health field.

EXPERIENCE:

Entry level (no prior related work experience)
