About the Role

The Placement Coordinator (PC) works under the general supervision of the Admission/Placement Coordinator. The PC facilitates the Community Living Discharge Planning (CLDP) process and develops transition plans for individuals moving from State Supported Living Centers to community settings. The PC serves as a resource to interdisciplinary team members, community providers, and local authority staff, providing expertise in the community transition process.

Additional duties may include training others, preparing policy and procedure manuals, and participating in disaster response or continuity of operations activities.

Essential Job Functions
  • Attend work on a regular basis, following a scheduled shift or rotating schedule, and as required, extend shifts or work overtime.
  • Consult with facility professionals and interdisciplinary team members to identify needed supports and services for individuals referred for community transition.
  • Serve as a liaison to staff, community organizations, and the general public to explain program specifics and requirements.
  • Participate as a resource in interdisciplinary planning meetings related to community referrals and transition planning.
  • Ensure timely and thorough completion of required documentation within specified timelines.
  • Assist in scheduling and ensuring in‑services are provided to potential providers before trial visits and community transitions.
  • Review discipline transition assessments and develop a draft transition plan for distribution to the individual, LAR, interdisciplinary team members, community provider, and LA in preparation for the CLDP meeting.
  • Collaborate with disciplines to address inconsistencies or issues identified in assessments.
  • Schedule and facilitate CLDP meetings, identifying community supports and services; assure completion and distribution of the CLDP prior to transition.
  • Assist in developing policy and procedure manuals, administrative reports, studies, and specialized projects.
  • Train others as assigned.
  • Participate in disaster response or continuity of operations activities, which may require alternate shift patterns or location assignments.
Knowledge, Skills, and Abilities
  • Prepare reports and communicate effectively orally and in writing.
  • Work professionally with individuals with intellectual disabilities, families, staff, and community providers.
  • Knowledge of DADS community programs including ICF/ waiver programs.
  • Gather, assemble, correlate, and analyze facts and devise solutions to problems.
  • Computer skills in Word, Excel, and other Microsoft Office products.
  • Effectively handle multiple responsibilities and plan work activities.
Initial Screening Criteria
  • Bachelor’s degree with a major in a human services field such as psychology, sociology, social work, special education, rehabilitation counseling, therapeutic studies (e.g., music, art, occupational, physical therapy), or recreation.
  • One year of experience working with individuals with intellectual disabilities.
  • Experience working in Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) and Home and Community-based Services (HCS) waiver programs is preferred.
