About the Role

Join Texas Health and Human Services as a Placement Coordinator (Program Specialist I) to support individuals with intellectual and developmental disabilities transitioning from state‑supported living centers to community settings.

Key Responsibilities
  • Facilitate the Community Living Discharge Planning (CLDP) process and develop transition plans.
  • Consult with Qualified Intellectual Disability Professionals and interdisciplinary team members regarding needed supports for individuals referred for community transition.
  • Participate in planning meetings, ensuring timely completion of required documentation within specified timelines.
  • Assist in scheduling and providing in‑services to potential providers prior to trial visits and community transitions.
  • Review discipline transition assessments and develop draft transition plans for distribution to individuals, LARs, team members, community providers, and local authorities.
  • Schedule, facilitate, and support the planning and development of CLDP meetings, identifying community supports and services for transitions.
  • Assist in developing policy and procedure manuals, preparing administrative reports, studies, and specialized projects.
  • Train others and perform related duties as assigned, including disaster response/COOP activation as required.
Qualifications

Education: Bachelor’s degree in human services (psychology, sociology, social work, special education, rehabilitation counseling, therapeutic studies, or recreation). Experience: One year working with individuals with intellectual disabilities; preferred experience in Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) and Home and Community-based Services (HCS) waiver programs.

Knowledge, Skills & Abilities
  • Prepare reports and communicate effectively orally and in writing.
  • Work professionally with individuals with intellectual disabilities, family members, staff, community providers, and others.
  • Knowledge of DADS community programs including ICF/ waiver programs.
  • Analyze facts, identify inconsistencies, and devise solutions to problems.
  • Computer skills in Word, Excel, and other Microsoft Office products.
  • Effectively handle multiple responsibilities and plan work activities.
Additional Information
  • Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, Employee Misconduct and HHS List of Excluded Individuals/Entities).
  • Required random drug testing for all State Supported Living Center employees.
  • Flexibility in work hours may be required; position may require overtime, extended hours, weekends, and holidays.
  • Must be at least 18 years of age.
  • Males 18–25 must register with the Selective Service.
  • This position may have telework options per agency policies.
