The Sisters of St. Joseph Brentwood are seeking a Social Worker to provide person-centered, mission-aligned services across three LWA sites:
Brentwood, Hempstead, and Far Rockaway.
You will coordinate psychosocial care, benefits, and transitions among hospitals, home care, and skilled nursing, collaborating with sisters, clinicians, and providers to support dignity and autonomy. The role requires an MSW and NY LMSW/LCSW license, with strong assessment, advocacy, and crisis-intervention skills.
