Shalom Hospice is seeking a registered nurse to plan, organize and direct hospice care within patients’ homes and communities. The role emphasizes community health education and collaboration with a multidisciplinary team to meet patient and family needs.
The RN will conduct comprehensive assessments, develop and adjust care plans, administer medications, provide patient education, and coordinate care with physicians and other professionals. A valid RN license and CPR certification are required.
