What You'll Do:

This position is responsible for providing direct care coordination ensuring the care delivered is effective, efficient and achieves desired patient outcomes. This position will work closely with the clinical staff in the clinic to implement coordination of patient care for their behavioral health needs. The Registered Nurse is responsible for providing clinical nursing care to clients according to established professional nursing practice standards of the American Nursing Association and in accordance with the North Carolina Nurse Practice Act and other regulatory standards.

Responsibilities include:

  • Provide care coordination services for patients in the medical and behavioral health setting and the Medication Assisted Treatment program
  • Provides medication injections as needed
  • Understands and follows established policy and procedure under minimal supervision as well as makes decisions within established clinical nursing standards and protocols
  • Manages multiple priorities by listening, assessing, speaking and transcribing patient information in the computer
  • Facilitates referrals to primary care providers, specialists, healthcare facilities, and community resources and communication with providers and healthcare organizations as appropriate
  • Demonstrates problem-solving skills, utilizes protocols and initiates solutions to facilitate the telephone encounter to an appropriate and timely disposition
  • Acts as a liaison between providers, care coordinators, hospitals, urgent care facilities, and other health care providers
  • Provides patient-centered care and education focused on individual and family engagement and behavioral change, treatment compliance and health risk factor reduction
What We're Looking For:
  • Graduate of an accredited BSN nursing program;

    Experience:

    1 year nursing experience in primary care and/or public health setting preferred;
    Experience in recovery work
  • Evidence of excellent oral and written communication, high level of current clinical knowledge
  • Strong interest in working with an interdisciplinary healthcare team, culturally diverse patient populations, and serving as a preceptor for student learners
  • RN currently licensed to practice in North Carolina
