Care Partners Health Services is seeking a Primary Registered Nurse Case Manager for Home Health in western North Carolina. You will provide direct patient care per physician orders, follow the plan of care, and work closely with the care team to achieve positive patient outcomes.
Responsibilities include coordinating hospice services, communicating needs to the interdisciplinary team, and managing pain and symptom control to ensure comfort and quality of life for patients and families.
