Provides coordinated end-of-life nursing care for patients in home or facility settings.
- Conducts comprehensive patient assessments and develops individualized plans of care.
- Manages symptoms and supports comfort-focused outcomes.
- Reviews and updates care plans to reflect patient needs.
- Educates patients and caregivers on hospice philosophy and available treatment options.
- Collaborates with the interdisciplinary team to coordinate services and resources.
- Supports families through care transitions and the hospice experience.
- RN licensure and hospice/end-of-life nursing experience (preferred).
- Strong clinical assessment, care planning, and symptom management skills.
- Excellent communication and teaching abilities for patients and caregivers.
- Ability to work effectively within an interdisciplinary team and provide compassionate support.
