FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one in-home and senior living care in New Jersey. You will evaluate patients, develop individualized plans of care, and deliver evidence-based treatment in patients’ homes or living communities.
You will document in FOX’s EMR, collaborate with an interdisciplinary team, and educate patients and caregivers to promote safety, function, and aging in place.
