-Oak Tree Surgery Job Summary The Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to patients registered at Oak Tree Surgery. Working as part of a multidisciplinary primary care team, the post-holder will provide holistic care, support long-term condition management, promote health and wellbeing, and contribute to quality improvement initiatives within the practice.
- Deliver chronic disease management clinics (e.g., diabetes, asthma, COPD, hypertension). (training provided)
- Administer childhood and adult immunisations in accordance with national guidance.
- Undertake cervical cytology screening.
- Provide wound care management, including dressings and suture removal.
- Carry out health checks and other preventative health screenings.
- Perform travel health assessments and vaccinations (where trained). Provide lifestyle advice and health promotion interventions.
- Recognise and respond appropriately to safeguarding concerns.
- Maintain accurate, contemporaneous patient records in line with professional standards.
- Assist in delivery of research projects (training provided)
- Deliver patient-centred care that respects dignity, diversity, and confidentiality.
- Provide clear health education and self‑management support.
- Communicate effectively with patients, carers, and the wider healthcare team.
- Support patients in shared decision‑making about their care.
- Contribute to QI projects and enhanced service delivery.
- Participate in clinical audit and quality improvement initiatives.
- Maintain compliance with infection prevention and control policies.
- Work within NMC Code and professional scope of practice.
- Participate in significant event analysis and learning processes.
- Work collaboratively with GPs, healthcare assistants, pharmacists, and administrative staff.
- Support mentoring and supervision of junior staff or students (where appropriate).
- Attend and contribute to practice meetings.
- Essential:
Registered Nurse with current NMC registration. - Competence in immunisations and cervical cytology (or willingness to train).
- Experience in long‑term condition management (or willingness to train).
- Excellent communication and organisational skills.
- Ability to work independently and as part of a team.
- Desirable:
Experience in primary care. - Experience with EMIS clinical system.
- Independent prescribing qualification.
- Professional Development:
The post-holder will be supported in maintaining professional registration and continuing professional development (CPD). Opportunities to develop specialist interests aligned with practice needs will be encouraged.
