Position: Community Mental Health Nurse (RMHN)

Overview

To work within a multidisciplinary team and provide care and support under the Mental Health Measures 2010 for people with complex mental health conditions. This role requires a multi-professional approach and effective communication. Robust mental health assessments and risk management, including initial assessments and extended assessments to establish a treatment pathway for the individual. Communication from family and carers is essential, with collaborative work on treatment options, care and treatment plans, and safety plans.

The CMHT Bridgend operates 7 days a week, with one staff member working the weekend with HTT support.

Responsibilities
  • Care coordination under the Mental Health Measures Wales.
  • Plan care, setting goals, manage risk and risk formation.
  • Holistic care and risk management; communicate with patients, carers, family, and multi-professionals (e.g., Children services, MARAC, police, GPs, third sector, LMPHSS, HTT, MASH).
  • Documentation and ongoing referrals; complete CMHT, MHA documentation/referrals as needed (e.g., Tribunal reports, CTP, WARRN, assessment tools, daily notes, ongoing referrals).
  • Lone working with safety measures in place to protect staff.
Qualifications and Skills
  • Welsh Skills Desirable:
    Not essential but advantageous for shortlisting. Speaking & Listening at Level 3 (CEFR B2) or above; basic conversations with patients about everyday health.
