Commissioning for Complex Needs Team

Are you an experienced Social Worker who is passionate about supporting people to achieve meaningful outcomes, maximise their independence and live the life they choose?

We are looking for a motivated, creative and forward-thinking practitioner to join our Commissioning for Complex Needs Team.

This is more than a traditional assessment and care planning role. As part of our Commissioning for Complex Needs Team, you will support adults with complex needs to build on their strengths, aspirations, relationships and community connections, helping them achieve what matters most and live well.

About the Role

At the heart of this role is progression.

We believe support should create opportunities for people to progress, build on their strengths and achieve the outcomes that matter most to them.

Using strengths-based and outcome-focused social work practice, you will work alongside individuals, families, carers, providers and partner agencies to develop progression plans that promote independence, wellbeing, choice and control. Bringing professional curiosity, creativity and critical thinking to complex situations, you will help identify opportunities, overcome barriers and support people to achieve their goals and aspirations. Through assessments, reviews and collaborative working, you will help ensure people have the right accommodation, care and support to achieve the outcomes that matter most.

You will work closely with providers and partners to ensure support remains focused on progression, enabling people to develop skills, build confidence and move closer to living the life they want to live.

You will also play an important role in supporting access to NHS Continuing Healthcare (CHC) and other funding arrangements, helping to ensure people receive the right support at the right time.

Alongside your direct work with individuals, you will contribute to developing solutions for people with complex needs and help inform future accommodation, care and support options, ensuring services continue to evolve to meet changing needs.

What We're Looking For

We are looking for someone who:

  • Is a qualified Social Worker registered with Social Care Wales.
  • Has experience of working with adults with complex and multifaceted needs.
  • Is committed to strengths-based, person-centred and outcome-focused practice.
  • Is professionally curious, creative in their approach and passionate about helping people realise their potential.
  • Understands the importance of progression, independence, choice and control in people's lives.
  • Has experience of, or expertise in, NHS Continuing Healthcare (CHC) and effective partnership working across health and social care.
