Adults Social Worker – Integrated Network Team Bridgend Agency | Initial 3 month contract Full time preferred Up to £35.32 per hour Hybrid working – 2 days home / 3 days on site
Bridgend Council is looking for an experienced Adults Social Worker to join their East and West Integrated Network Teams on initial 3 month contracts. Full-time candidate required but happy to receive applications from those considering part time. The East is based at Glanrhyd Hospital, Bridgend.
You will be working as part of a well-established multi-disciplinary team supporting adults aged 18+ with frailty, long-term conditions, disabilities and often additional mental health or emotional wellbeing needs.
The role:- Adults community social work within an Integrated Network Team
- Managing complex cases with a strengths-based, person-centred approach
- Care coordination and multi-agency working
- Regular visits required (driver essential)
- Hybrid working - 2 days from home, 3 days on site
- Qualified Social Worker with post-qualifying adults experience
- Registered with Social Care Wales
- Confident working within an MDT environment
- Strong assessment, care planning and care coordination skills
- Able to work autonomously while contributing to a wider team
- Up to £35.32 per hour, dependent on experience
- Initial 3 month contract

To apply for this role, you will need to hold -
- A minimum of 6 months experience of working in the UK as a registered social worker
- A relevant Social Work Qualification
- Registration with Social Care Wales / Social Work England
- Enhanced DBS, registered on the update service or be happy to apply for a DBS
- 3 Years of referencing
- Driving license with access to a car
- 20 years’ experience of the Social Work market
- Your local agency based in Cardiff
- Welsh-speaking consultant available should you wish to converse in Welsh throughout your job search
