Summary:

Provides casework and counseling services to Residents and their families.

Shift: Days

Essential Responsibilities:

• Experienced with skilled nursing (LTC), assessing the needs of patients/residents and families, providing support and guidance, developing and coordinating discharge plans, referrals to appropriate agencies, MDS, Care Planning.
• Facilitate discussion with resident/patient, responsible parties and staff regarding concerns. Facilitate meetings with responsible parties and support groups as necessary. Make notations in medical record. Maintain complaint log.
• Participate in designing, and on occasion present, in-services to Mozaic Senior Life staff regarding issues related to resident rights and psychosocial needs of the elderly. Supervise graduate and undergraduate students in social work and/or volunteers as time permits.
• Coordinate and complete investigation of any abuse allegations submitting written reports as requested.
• Represent the resident/patient in court matters when necessary.
• Other related duties as assigned.

Minimum Qualifications:

• Education:
  
  Master's Degree in Social Work preferred.
• Experience:
  
  1-3 years clinical experience with the elderly population preferably in a LTC setting.
• Licensure: MSW preferred.

Ability to handle multiple demands and deadlines;
Ability to communicate effectively;
Ability to utilize various software packages to carry out position responsibilities.

Salary: $30.00 - $36.00 per hour
