Job SummaryThe Family Navigator will utilize working knowledge of local social service resources to provide case management in tandem with clinician treating families who are referred by DCF.
- Must be comfortable working with clients and families in home as well as office setting and transporting clients when needed.
- Meet with families and individuals regularly both in-home and in-office to assess and manage case service needs and safety plans.
- Research and connect with local social services and other resources to proactively problem-solve and help Clients meet a variety of basic and higher-level needs, including but not limited to connecting Clients to resources for housing, education, childcare, benefits, clinical services, parenting services, recreation, etc.
- Work as a team with a clinician, program manager and DCF to provide coordinated and effective treatment.
- Arrange or provide transportation for Clients as necessary.
- Attend weekly, monthly, and quarterly program meetings and trainings in various CT locations.
- Maintain clear and concise records and documentation according to policies and procedures
- Flexibility required to meet client needs. This position may require working some evenings and weekends.
- Language Ability:
Very high. Strong written and verbal skills in English. Ability to speak Spanish a plus. - Math Ability:
Medium. Must be able to provide and interpret statistics and budgets - Reasoning Ability:
Very High. Must be able to coordinate multiple needs simultaneously and problem-solve innovatively. - Computer
Skills:
Very high. Must know Word, Excel, Outlook, be comfortable with iOS (iPad/iPhone), and utilize online assessment services and electronic records per training provided. - Must be willing to work with clients in home settings across Fairfield County.
Education and Experience
- Education/
Experience:
Bachelor’s Degree in Human Services Field and 3 years’ experience providing direct services to families with complex needs. Experience with intimate partner violence a plus. - Services to be performed both in center-based and in-home settings as needed.
- Must have valid license and own vehicle.
