The Registered Nurse (RN) - Home Health Admissions is responsible for completing start-of-care (SOC) and admission visits in patients' homes. This role conducts comprehensive in-home assessments, completes required documentation (including OASIS, as applicable), and collaborates with the interdisciplinary team to ensure safe, compliant, and patient-centered home health services.
Key Responsibilities- Complete home health admission and start-of-care visits in patients' homes.
- Perform comprehensive nursing assessments, including physical, psychosocial, and environmental evaluations.
- Accurately complete OASIS assessments and all required admission documentation in compliance with CMS and agency standards.
- Collaborate with physicians, therapists, and interdisciplinary team members to develop individualized plans of care.
- Educate patients and caregivers on diagnoses, medications, treatment plans, safety, and disease management.
- Administer medications and treatments as ordered and within scope of practice.
- Monitor patient status and promptly communicate changes in condition to the physician and care team.
- Coordinate services, referrals, and follow-up visits to ensure continuity of care.
- Adhere to infection control, safety, and documentation standards in the home setting.
- Active Registered Nurse (RN) license in the State of Connecticut.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 1 year of nursing experience; home health, admissions, or start-of-care experience preferred.
- Current Connecticut driver’s license with reliable transportation and ability to travel within assigned service area.
- Knowledge of OASIS-E documentation and CMS home health regulations strongly preferred.
- Familiarity with Connecticut Department of Public Health (DPH) regulations for home health agencies.
- Strong clinical assessment, critical thinking, and time-management skills.
- Excellent communication and interpersonal skills with patients, families, physicians, and interdisciplinary team members.
- Ability to work independently in the field while collaborating with a multidisciplinary team.
- Proficiency with electronic medical records (EMR) and mobile documentation systems.

- Salary Range: $79,000 - $93,000 annually (commensurate with experience and qualifications) per Connecticut pay transparency law.
