The RN will provide in-home behavioral health nursing services, create care plans, and coordinate with hospitals and primary care physicians to support seniors and clients with disabilities. Requirements include an RN or BSN, CT license, ability to provide essential documents (license, SSN, ), and the capacity to complete a quick physical/PPD.
