Position Summary
The Bilingual Case Manager will provide counseling services for children living in our temporary emergency shelters and homes as they awaitfamily unification. The Case Manager is a motivated, culturally sensitiveindividual who is dedicated to the care of international children living in the United States. The Case Manager will coordinate and provide individual andgroup living skills assessment and training for all children within the Juntosprogram.
Duties and Responsibilities
The Case Manager assists in maintaining 24-hour case management coverage and provides rotating on-call emergency coverage asneeded. This individual coordinates and provides individual and groupliving skills assessment and training. The case manager monitors and performsindividualized life skills instruction, designs a Sponsor reunification planfor each youth assigned to caseload and updates the plan on a weekly basis. The Case Manager performs direct counseling and treatments services and performscase management for youth assigned in caseload while also assisting in otheradvocacy for sheltered youth as needed.
The Case Manager transports youthin caseload at the onset of a youth's shelter placement and to various medical,recreational, educational and employment appointments. The Case Manageradvocates for youth and performs crisis intervention, counseling, youthsupervision and other related activities to meet 24-hour operational activities of the program. The schedule for this position is generally 9am-5pm,Monday-Friday. However, the Case Manager is on call 24/7/365 and the position requires some evening and weekend hours.
Requirements:
- Certified Social Worker (CSW) or equivalent certification, if applicable
- Bachelor of Social Work (BSW) degree or related field, or extensive experience
- Threeto five years' experience working with youth
- Twoyears of experience counseling high-risk youth and families, one year experience in providing group counseling services
- Bilingual fluency in English and Spanish
- Excellentdriving skills and current driver's license with a driving record of atleast three years
- Vehicle with current insurance and registration
